Job summary
The Facility Linkage Supervisor will be responsible for working with high volume
health facilities and community structures to contribute to improved linkages
along the continuum of care and improved retention of all children living with
HIV and Adolescents living with HIV at supported high volume health facilities
in Kampala. S/he will coordinate activities to support intra/inter-facility linkages
and facility–community linkages and other USAID Implementing Partners (IPs).
Responsibilities and duties
1. Coordinate linkages to care for Children Living with HIV and Adolescents
Living with HIV at supported facilities and other community support
services as well as organizing inter-facility viral load meetings.
2. Promote real-time follow-up for all Children Living with HIV and
Adolescents Living with HIV with missed appointments and those eligible
for ART initiation to ensure continuity of treatment and avoid treatment
interruptions.
3. Identify eligible CLHIVs, ALHIV, HIV exposed Infants (HEI) and children
of non-suppressed caregivers and refer them for assessment and enrollment
onto USAID ICYD Activity.
4. Work with facility-based linkage facilitators, health facility in-charges, ART
in charges and other staff to document and update all relevant registers and
CLHIV/ALHIV client files.
5. Organize facility-based case conferences and strategies to ensure viral load
suppression among CLHIV/ALHIVs and data management systems.
6. Support Young Adolescent Peer Support activities (YAPs) with the YAPs
facilitators in the supported health facilities.
7. Submit weekly and monthly data reports to the case manager through the
Division Coordinator.
8. Work with the health workers to conduct home-based index contact tracing
for HTS and referral for ART initiation.
9. Support and observe the safeguarding culture of ROM, regularly
documenting and reporting identified and potential safeguarding issues; Ensuring health, safety, and security in your area of operation, as you
promote the vision, mission and the values of Reach Out Mbuya.
10. Any other duties assigned by the immediate supervisor.
Qualifications, Skills and Experience:
• The applicant must hold a Bachelor’s degree in Social Work and Social
Administration, Development Studies.
• Postgraduate training and experience in the field of HIV/AIDS and previous
work with PEPFAR funded projects is an added advantage.
• At least 3 years’ experience in HIV and TB screening, counseling, testing
and linkage services in a busy clinical setting.
• Understanding of the importance of the universal test and treat (UTT) policy,
notably linkage to care and retention of clients in care.
• Training in Differentiated Service delivery models (DSDM)
• Extensive experience with client tracking (via telephone and in-person
through home visits and technical Knowledge of HIV/AIDS strategies
including the UNAIDs 95, 95 95 HIV Cascade.
• Demonstrable teamwork, strong interpersonal, oral and written
communication skills are key.
• S/he should have computer skills including MS Word and Excel.
